Update tools/moztreedocs/__init__.py to read docs/index.md and write
it to staging as index.md. Adjust format_paths to use plain newlines
(no leading indent) since MyST toctree fence-block content is parsed
verbatim. Escape Python str.format placeholders ({{toctree}}, {{ref}})
in the new index.md.
