Joe Drew fae510a6de Bug 393936 - nsIRequest::cancel() must not notify anything sync. Make removal from the loadgroup async, and add another call cancelAndForgetObserver() that removes the listener immediately (since some callsites expect that). Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell.
