Removed GetBaseURL, rename GetMedium to Medium and inline it, remove virtual style resolution methods that forward to style set (callers will call style set methods directly). bryner@brianryner.com 640685cbe9 Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. jaggernaut@netscape.com baba14c5c8 Bug 88413: Remove |GetUnicode()| from nsString (and replace it with |get()|). r=dbaron, rs=scc. 
This removes all call-sites I can currently fix. Tomorrow I'll try to get someone to checkin my changes to security/ and I'll get some help with the Netscape side of things.

nsString::GetUnicode()'s final death-blow will be dealt soon. Please keep this in mind as you add new code :-)

pollmann@netscape.com b050d1d105 Bug 77834: Fix problem where form control state is being restored to the wrong form control or not restored at all when going back and forth through session history. The problem was that the hash key we used was the content ID, which was not consistent from page load to page load.
