Normalize header names/values in executor.sub.html

To check the headers from ServiceWorker FetchEvent (from
https://fetch.spec.whatwg.org/#headers) in
https://chromium-review.googlesource.com/c/chromium/src/+/6364354 and
the headers from WPT server (in `executor.sub.html`) in the same way,
this CL changes the format of the header information in
`executor.sub.html` so that:

- Keys are the lower-cased header names, and
- Values are unescaped, and
- Entries doesn't exist when the corresponding header doesn't exist
  (instead of having an entry with empty string value).

`sec_speculation_tags` is left inconsistent for now (as the
`sec_speculation_tags` and this CL is landed individually and not yet
merged), and to be fixed by the mid-term fix for crbug.com/404573971.
